I don't recall Karma ever having a confirm after selection, it't not part of the karma feature that I can find but I'll look into.

Tried sending a private message from someones profile page. Instead gave them a -1 karma and still cant access send a message.

Please but a "confirm change in karma" yes / no

I just came on here to report a 404 error and saw this so thought no need, but then I saw you also said it's resolved.

I just hovered over the "Forum" tab at the top and clicked on "recent posts" and received a 404 message.

I then had to scroll down and follow a link on the left to one of the "forum topics" and then I could click on the "recent posts" tab at the top which worked, but going from the main "forum" tab at the very top threw me an error.

ok that was quicker then expected. Looks like its solved so should be nice intelligent URLs now. Will still work on it a bit but getting there.
